Advance Care Planning
This page provides information about creating an advance care plan so that your loved ones will be able to carry out your wishes if you are no longer able to communicate. An advance care plan includes your preferences for medical care and your designation of a person to make those decisions for you. It may also include your preferences for comfort, interaction with your family and friends, or things you want your loved ones to know. The video below explains more and identifies three documents that meet Tennessee legal requirements and do not require you to hire an attorney.
